Audit of Community Health Service Provision for Wheelchair Users in Ireland



Dear sir / madam,

I am conducting a survey of the accessibility of community health services to wheelchair users in Ireland.

The purpose of the survey is to identify difficulties you have encountered accessing your GP Surgery, Dental Surgery, Local Pharmacy and General Hospital in the hope that this will lead to increased awareness of the needs of wheelchair users and ultimately improved service provision in Ireland.

Your identity is confidential and will not be revealed by your participation in the survey.
