Fenway CDC Community Needs Assessment

 
This is a needs assessment survey for the Fenway community. The goal of the survey is to learn what local services are currently being utilized by Fenway residents, in the neighborhood and nearby, and what resident services are lacking in the Fenway neighborhood. Some examples of local services include: daycare, after-school programs, social and recreational events, job and educational training, senior programming and health services. Your input is greatly valued and will help the Fenway CDC plan for a future community center in the neighborhood. Note: Some questions are required while others are optional.
